Add upper layer terms

Related to #5 (closed). Added upper level terms to external_import.tsv and defined some cross ontology subclass relations in cpont.tsv.

Removed the CTNNB1 mutation terms and the APC Gene Mutation term since they are the wrong terms for the OMOP measurement concepts they were intended to represent.
